5/16/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Ben-Nita appeared to be in positive mood evident by facial expressions such as a smile on the face. QMHP-C-C engaged mother in a discussion to determine her behaviors since the last session. Mother reports Ben-Nita continues to struggle with being able to get along with her siblings. Behavior: Ben-Nita displayed withdrawn behaviors at times within the session when discussing her behaviors with her mother. Ben-Nita at times got loud with QMHP-C-C and her mother and was redirected within the session. Intervention: QMHP-C processed Ben-Nita positive behaviors. QMHP-C assisted Ben-Nita in developing more social distancing skills. QMHP-C inquired about possible challenges Ben-Nita can experience with applying this skill. QMHP-C provided positive feedback. QMHP-C applauded Ben-Nita for making the big decision to want to work on changing her behavior. QMHP-C discussed Ben-Nita the importance of working hard to make sure that she continues to stay focused on her goals. QMHP-C challenged Ben-Nita to continue displaying a positive attitude while employing positive reinforcement when Ben-Nita is able to achieve her goals and objectives. QMHP-C provided praise when the Ben-Nita continued to display positive behaviors throughout the day, remaining on task and was very respectful to all adult figures. QMHP-C isolated Ben-Nita from distractions to decrease disruptive behaviors and to reduce excessive talking. Intervention: QMHP-C guided Ben-Nita by asking her why she believed her family was important to her and what is the overall image that she wants to depict when it comes to her family. QMHP-C assisted Ben-Nita in creating goals for her family and understanding what role would be in achieving them. QMHP-C role played with Ben-Nita and demonstrated for positive behaviors and responses to increase positive interactions between her and the family. QMHP-C applauded the behavior of Ben-Nita and recognized how her and her mother had been interacting positively. QMHP-C reviewed an article with the two that discussed relationship skill building and ways to better a relationship with one another. QMHP-C engaged in conversation with Ben-Nita about positive decisions and her plan to continue building relationships from home. Response: Ben-Nita showed her gratitude for being able to release physical tension stating that she felt relaxed. Ben-Nita was able to cooperate and comply to rules and regulations due to positive reinforcement provided by the QMHP-C. Ben-Nita implemented pro social skills successfully. Ben-Nita has presented herself in a responsible way and is making progress towards her goals and objectives. Ben-Nita was able to utilize coping skills to eliminate stressors and unwanted feelings. Ben-Nita displayed self determination to stay focused and pay attention in the home. Response: Ben-Nita utilized meditation and relaxation techniques. Ben-Nita was seen decreasing anger and frustration. Ben-Nita reported a decrease in symptoms following the relaxation techniques. Ben-Nita is making progress with expressing her feelings and showing others respect and courtesy by talking with appropriate tone and initiating conversation. Ben-Nita was able to speak calmly and demonstrate her improved communication skills while engaging in self-reflection with QMHP-C and mom.
